Concusion Release
This acknowledgment explains the risks of concussion and mild traumatic brain injury (MTBI), the responsibility to report symptoms, removal-from-play requirements, medical clearance for return-to-play, and the potential for long-term neurological effects associated with football and cheer participation.
Parent Code of Conduct
This Parent Code of Conduct outlines expectations for parent and guardian behavior, including sportsmanship, sideline and event conduct, communication procedures (including the 24-Hour Cool-Off Policy), alcohol and substance policies, social media conduct, and the authority of Clovis Clash to remove individuals or impose disciplinary action for violations.
Wavier of Liability
This waiver explains the inherent risks of participation in youth football and cheer, including physical injury, concussion, long-term neurological risks, weather-related conditions, and volunteer-led instruction. It also includes assumption of risk, medical authorization, release of liability, and acknowledgment of program rules and safety decisions.
